Welcome to Heritage Reservation Winter Staff

There are several things that you need to accomplish before arrival at Heritage for the winter weekends. Please pay close attention to the items listed here and make sure to complete all that is applicable to you and your position. We look forward to seeing you all at Heritage in January and February!

Paperwork - Due Prior to your first weekend.

All paperwork is due to Flag Plaza, Attn Michael Crane, prior to your first weekend.

Most forms are fillable PDFs and all forms can be downloaded via this link.

Online Training

There are several online training courses that are required in addition to the training at staff weekends and staff week. Please have a close look at the chart and see that you complete what is required for your position. Most of these courses take less than an hour but the Boaters Safety Course is several hours long so prepare your time to be completed before camp.

      • Go to: http://myscouting.scouting.org
      • Create an Account and Sign-In. Choose E-Learning by clicking the link in the left column.
      • Start and complete the training(s) for your position.
      • Print the completion card and bring it to camp. If you do not have this card, you will be required to retake the training at camp. You can login at anytime and print completion cards so please remember you login information.

Heritage Reservation Staff Employment Forms

All paperwork is due to Flag Plaza, Attn Michael Crane, prior to your first weekend. Most forms are fillable PDFs.

A note to those also applying for summer camp staff: New employment paperwork must be completed after a lapse of 30 days. You may want to save a copy of these forms after you have filled them out, prior to signing and dating. When turning in summer camp paperwork, recheck to ensure accuracy and that no information has changed and then sign, date and submit.

Items to Return to Flag Plaza

  • Letter of Agreement - Sign and have parent/guardian sign if under 18. Keep one copy for your records.
  • Code of Conduct - Sign and have parent/guardian sign if under 18. Keep one copy for your records.
  • New Hire Form - Fill in bold fields and select gender and marital status.  The rest will be completed by the Accounting department.
  • Direct Deposit - Your paycheck may be deposited into a checking or savings account, or you may choose to not use direct deposit.  Please sign and fill out information in the middle and bottom of the form:
    • Checking: Attach a voided check
    • Savings: Attach a letter from your bank with your name, routing number and account number, not a deposit slip
    • No direct deposit: Ignore this form
  • W-4 - Please ensure lines 5 and 6 are completed if not exempt
  • I-9 - Copies of appropriate forms must be included or brought to Flag Plaza for verification.  One item from List A or one document from Lists B and C are needed.
  • Local Earned Income Tax - Find your PSD by using this web app. Enter Work Location as 2690 Dinner Bell Five Forks Road, Farmington, Pa. 15437
  • City of Pittsburgh - Fill out form including checking one of the reasons for exemption (choices 1-4)
  • City of Pittsburgh residents - Fill out only if you are a resident of the City of Pittsburgh, otherwise, discard.
  • O'Hara Township residents - Fill out only if you are a resident of O’Hara Township, otherwise, discard.
  • Boy Scouts of America Adult Application - needs to be completed annually by each paid camp staff member.
  • Staff Permission - For all staff members under the age of 18

  • Work Permit - Any camp staff member under the age of 18 must have a work permit.  Materials can be obtained at your school guidance office. The required council signature can be obtained by visiting Flag Plaza, or mailing the form to Mike Crane at LHC-BSA, 1275 Bedford Avenue, Pittsburgh PA 15219. You must bring your work permit to camp with you.
